Recording and Storing Information

  • Whether you work with individuals, couples or groups, this system provides you with a complete service for recording and storing notes and files.
  • If you use a screening instrument (e.g. CORE, DASS, PHQ9), then the score is recorded each time you use the instrument and Sincharta will track all the changes over time; e.g. it provides an automatic comparison between Beginning + End Scores.
  • If you have supplementary files you want to upload or later additional reflections linked to the work these are accommodated too
  • All notes are “date stamped” which provides evidence of when notes and observations have been recorded.
  • As well as clinical notes you may also want to keep demographic data (e.g. age, gender, marital status, presenting issue).
Sincharta not only allows you to store this information but it will provide you with reports which gather this information together in meaningful ways – see 'Generating Reports'

Sincharta not only stores your notes and files, it also supports practitioners because it prompts the user to follow good practice by checking whether simple key questions have been asked;

For example,

Did you explain the meaning of "confidentiality" to the client?
The user is free to answer “yes” or “no” but at each session the question will be there until the practitioner confirms that they have.

Did you agree a contract with the client indicating a number of sessions followed by a review session?
Once again the user is free to answer “yes” or “no” but at each session the question will be there until the practitioner confirms that they have agreed a contract.

If you agreed to carry out a review session, at which session will this take place?
When the user confirms the session when a review is due then Sincharta will remind the user when the review session is coming up
